What are your regular office hours?

We are open Monday through Friday 9 am to 5 pm
You may call the office outside of business hours and leave a message, or, mail us at any time.

What information do I need to provide when scheduling an appointment?
  • patient’s name
  • patient’s date of birth
  • patient’s address
  • brief description of the reason for the appointment
  • home and alternate phone numbers
  • patient’s health insurance information
What should I bring with me to my appointment?

You should bring copies of all pertinent medical records such as doctors’ notes and laboratory and imaging results, including actual films. This information helps your doctor to understand your medical history, make a diagnosis, and suggest treatment options.

You should also bring your insurance card, as well as a referral and precertification information, if applicable. If you have chosen to complete the patient forms in advance, please remember to bring them to your appointment.

How early should I arrive to my appointment?

When you schedule your visit, our appointment coordinators will tell you when you should arrive and how you can prepare. In general, please plan on arriving 15 minutes prior to your scheduled appointment time.

Will someone call me to remind me about my scheduled appointment?

You will receive an appointment reminder call a day in advance.

I am new to Tavicare. Will I need to complete any forms?

Yes, you will be asked to review and complete various forms when you arrive at the practice such as:

Registration form

Patient consent form

Patient responsibility form

Many of these forms can be reviewed and completed prior to your appointment by visiting our patient forms.

What insurance plans do you accept?

Please note that Dr. Tavakoli does not accept direct reimbursement from insurance companies. Our office does provide a detailed invoice for clients who wish to be reimbursed by their insurance companies.

What is the “no-show” policy?

If you are unable to keep your appointment, please call your doctor’s office as soon as possible. If you miss your appointment and do not call to cancel, additional fees may apply.

Who can I call if I have a billing question?

For billing-related questions, please call (301-859-4400).

What forms of payments do you accept?

For your convenience, our practice accept cash, personal checks, and credit cards. Unfortunately, we do not accept traveler’s checks. We will charge your credit card only for the amount you authorize.

How can I get a copy of my test results and medical records?

To receive a copy of your test results and medical records, please call the office. You will need to sign a consent form to release this information if your records are being sent to another office or hospital.

What do I do if I run out of my medication?

We ask the prescription refills be completed during regular office visits whenever possible. Calls for prescription refills may be made 24 hours a day through our phone tree. Our Doctor must have the benefit of reviewing your medical records before prescribing any additional medications.calculate in advance when you may need a refill on medicines since some of the medications have to be ordered for you. When calling the office, Follow the prompt and choose your extension, Please have the medication name and dose, Pharmacy name and number available and leave your name and date of birth so we may expedite your request. Medications can be picked up at the office your can be shipped to your address.

What should I do if I miss my appointment?

We ask that you call the office during business hours to let us know if you can not make to your appointment 24 hours before. Otherwise this will appear as a no show and you will be charged with fees.

What do I do if the office is closed?

You may leave a message by phone. Non-emergency calls will be answered within 72 hours.
